New Student Registration
All New Student Registration and Residency Verification must be completed in person at the District Office at 203 N. Third St in Ashkum. Please call 815-698-2212 extension 2500 for availability. District Office summer hours are Monday-Thursday 8:00am – 3:00pm.
Items to bring with you when registering:
ONE of the Following:
- Mortgage Statement
- Homeownership Title or Deed
- Signed Lease & Proof of Payment
- Property Tax Bill & Proof of Payment or Escrow Documentation
Additional documents to bring:
- Birth Certificate
- Up-to-date-immunizations
- Completed Physical (Kindergarten, 6th, and 9th Grade)
TWO of the Following:
- Vehicle Registration
- Utility Bill (Electric, Gas, or Water)*
- Homeowners/Renters Insurance
- Driver’s License/Illinois ID
- Voter Registration
- Public Aid Documentation or Official Mail from Public Aid Office with current address
